ISO 45001: the standard and the certification

ISO 45001 is the international standard for occupational health and safety management systems. It gives organizations a structured way to identify hazards, reduce work-related injury and ill health, and demonstrate that safety is managed rather than assumed.

Published in 2018, it replaced OHSAS 18001 and became the first OH&S standard built on ISO's harmonized structure, the same architecture used by ISO 9001 and ISO 14001. That shared structure is why organizations running more than one management system can integrate them rather than maintaining parallel documentation.

The more consequential change was not structural. OHSAS 18001 could be owned by a safety function. ISO 45001 places overall responsibility and accountability on top management and makes it non-delegable. Organizations that treated the migration as a relabelling exercise have generally discovered this at their first recertification audit rather than before it. What the standard requires:

  • Systematic hazard identification and risk assessment across all work activities, including routine and non-routine work
  • Consultation and participation of non-managerial workers on defined matters, evidenced before decisions rather than after
  • Application of the hierarchy of controls in order, with documented reasoning where higher-order controls were rejected
  • Control of contractors, outsourced work and procurement, where serious incidents disproportionately occur
  • A maintained register of legal and other compliance obligations, and a means of evaluating conformity with them
  • Incident investigation that reaches root cause, corrective action, and demonstrable continual improvement

None of this is exotic. What makes ISO 45001 harder than it looks is that most of the evidence is behavioural rather than documentary, so a system can be complete on paper and still fail an audit conducted on the shop floor.

Who needs ISO 45001?

ISO 45001 is voluntary. No jurisdiction mandates certification. In practice it is driven by three forces: national OH&S law that certification helps you evidence, client and tender requirements, and insurers or parent companies asking for demonstrable safety governance rather than an incident count.

It applies to any organization regardless of size or sector, but demand concentrates where physical risk is high or contractor management is complex: construction, manufacturing, energy and utilities, logistics, chemicals, and healthcare. A second group certifies for commercial rather than operational reasons, typically service organizations that need it to qualify for tenders.

One practical note on scope. Multi-site organizations frequently define the OH&S management system around the sites they consider risky and leave offices, depots or subsidiaries outside it. Scope has to be defensible against the standard's own criteria, and an auditor who finds work activities excluded without justification will treat the exclusion as a finding rather than a design choice.

Switzerland

Swiss employers are bound by the Labour Act (ArG/LTr) and the Accident Insurance Act (UVG/LAA), with SUVA as the principal enforcement and insurance body and EKAS/CFST issuing the binding directives. The ASA/MSST directive requires employers to engage occupational physicians and safety specialists according to hazard level, and is the requirement most often satisfied informally and evidenced poorly. ISO 45001 is not mandatory, but it maps cleanly onto these obligations and gives you a single system that answers SUVA, your insurer and your clients at once. It is increasingly requested in construction and industrial tenders, and in Swiss supply chains serving EU principals.

European Union

Framework Directive 89/391/EEC obliges employers to assess and prevent occupational risks, transposed into national law across all member states with meaningful local variation in enforcement and documentation expectations. ISO 45001 is not required by that legislation, but its structure maps onto the directive's obligations closely enough that many organizations use it as the implementation framework rather than building one per country. For groups operating across several member states, that single-framework argument is usually the stronger business case, ahead of the certificate itself.

United Kingdom

The Health and Safety at Work etc. Act 1974 and the Management of Health and Safety at Work Regulations 1999 impose duties that ISO 45001 helps evidence, particularly the requirement for risk assessment that is suitable and sufficient. HSE does not require certification. Its practical weight comes from construction pre-qualification schemes and from board-level exposure under the Corporate Manslaughter and Corporate Homicide Act 2007, where the question is whether senior management arrangements were a substantial element in a fatality. A documented, audited system is the evidence that question turns on.

Lead Implementer or Lead Auditor: which path is right for you?

The choice follows your role in the management system, not your level of experience. Implementers build and run the system. Auditors evaluate whether it works. Both courses run four days, both are delivered onsite, virtual-live or self-study, and both include the official PECB exam.

Where implementers actually struggle

Clause 5.4, consultation and participation of workers, produces more findings than any other requirement in ISO 45001. The reason is a misreading that sounds reasonable: organizations point to a safety committee that meets quarterly and consider the clause satisfied. The standard asks for something narrower and harder. It names the specific matters on which non-managerial workers must be consulted, including how legal requirements will be met, what will be monitored, and how the audit programme is planned. An auditor will ask for evidence that consultation happened before the decision was made, not that workers were informed afterwards.

The second recurring gap is the hierarchy of controls in clause 8.1.2. It is a sequence, not a menu: eliminate, substitute, engineer, administer, then personal protective equipment. Many organizations reach for PPE and training first because those are fast and visible. The requirement is to document why the higher-order controls were rejected. A risk register that arrives at PPE without that reasoning is a finding waiting to happen.

A consistent pattern our trainers see in sessions concerns what actually changed with ISO 45001:

The migration from OHSAS 18001 was never really a documentation exercise, though most organizations treated it as one. The substantive change was that accountability moved. Clause 5.1 puts overall responsibility on top management and makes it non-delegable. In an audit interview this is usually apparent within two questions. If the operations director can describe the OH&S objectives and what changed because of them, the system is real. If the answer is that safety is handled by the safety manager, everything downstream tends to be documentation covering a gap.

Where auditors go wrong

An OH&S audit conducted from a meeting room is close to worthless, and it is the most common failure among auditors who transfer across from ISO 9001. Quality evidence is largely documentary. Safety evidence is behavioural, and it is visible only where the work happens. Procedures describe the intended method. Observation reveals the practised one, and the gap between them is the audit.

Sampling matters more here than in most standards. Night shift practice diverges from day shift, contractors work to different assumptions than employees, and a site audited only on a planned Tuesday morning shows you its best behaviour. Incident investigation records are the other tell. When root cause analysis stops at the worker not following the procedure, the investigation has stopped at the immediate cause and the system has learned nothing.

The second pattern concerns where the serious exposure usually sits, and it is rarely where organizations are looking.

Companies control their own workforce well because they see it every day. The contractor arrives on Monday, works to a different safety culture, and leaves on Friday. Clause 8.1.4 covers procurement and outsourcing for exactly that reason. A useful audit question is how a contractor was selected and what OH&S criteria applied before the contract was signed. Frequently nobody can answer, because procurement made that decision on price and never consulted the safety function.

What is changing next

Two developments are worth planning for. Psychosocial risk is moving from the margins into routine scope, supported by ISO 45003 as companion guidance, and the word health in occupational health and safety is increasingly read to include it. Separately, measurement is shifting. Organizations still reporting only lost-time injury frequency are reporting a lagging indicator that says little about whether controls are working and can be improved by reporting less. Clause 9.1 supports leading indicators, and auditors are asking for them.

ISO 45001 is also under revision as part of the same wave that produced ISO 14001:2026 and ISO 9001:2026, with no publication date confirmed. Certifying now carries no penalty. Revisions run multi-year transition periods, this wave is evolutionary rather than structural, and competence gained now applies to both the current text and the transition.
