What does ISO 9001:2026 clause 6.3 require when planning changes to the QMS?

Clause 6.3 of ISO 9001:2026 requires changes to the QMS to be planned, communicated, monitored and reviewed for results. You must give people the information they need, decide in advance how you will judge effectiveness, and check afterwards that the change worked. An existing project plan can carry this evidence.

Planning of changes existed in ISO 9001:2015, but it stopped largely at the decision to change: purpose, consequences, integrity of the QMS, resources and responsibilities. ISO 9001:2026 extends clause 6.3 across the full life of the change, so the requirement now reaches communication, monitoring and review of results.

For every significant change this means four things: the people affected get the information they need, the change is communicated to those who must know, its performance is monitored once live, and its results are reviewed against criteria set during planning. Deciding how you will judge effectiveness before the change goes live is the part most quality systems lack.

Take a new order-entry system. The plan should record the instructions users receive, who is informed, how errors will be tracked and when results will be reviewed. Auditors test the clause by sampling a recent change and asking for its rationale and review record, which quickly exposes updates made without documented reasoning.

Changes often come from actions on opportunities under clause 6.1, and closing gaps in 6.3 is a standard line in an ISO 9001:2026 gap analysis.
